One of the scarecrows in the field suddenly moves its head. Billy goes to look at it and it comes to life, grabs Billy and sprouts wings, disappearing into the sky with him, leaving Billy's family horrified and stunned. Returning home from a championship game, a group of high school basketball players, their coaches and cheerleaders become stranded on East 9 Highway in Kissel County when their bus breaks down. The trapped students and their coaches soon discover a makeshift shuriken has slashed their tires. The students learn of the fire to the church where The Creeper hid all the dead bodies while listening to the radio inside the bus. They attempt to limp home on the damaged bus, another makeshift shuriken blows up the remaining back tire. As night approaches, they see The Creeper (Jonathan Breck), a monster that resurfaces every 23 years to feed. The Creeper first kills the chaperones on the bus then targets the teenagers. Meanwhile, Jack and his son arm themselves and set out on a personal mission to hunt the Creeper down and to rescue some of the surviving teenagers.

Sinopse: A cada 23 primaveras, durante 23 dias, ele precisa comer. Estamos no 22º dia da fome bizarra da Criatura (JONATHAN BRECK) por pedaços de corpos, que cumpre um ritual. A antiga criatura que se alimenta de carne humana tem apenas mais um dia para se alimentar antes de desaparecer de novo, esperando pela próxima temporada de "caça". Taggart (RAY WISE) e os filhos, Jack Jr. (LUKE EDWARDS) e o caçula Billy (SHAUN FLEMING), trabalham em sua plantação de milho quando a fera alada ataca o mais novo, levando-o para um destino terrível e inimaginável. Inconformados, Taggart e o outro filho reúnem várias peças do equipamento da fazenda e carregam o caminhão em silêncio, preparando-se para um combate impossível. Eles ouvem o rádio de ondas curtas na esperança de obter alguma indicação do paradeiro da Criatura. No 23º dia, numa estrada deserta daquela região, um ônibus escolar transporta um time de basquete, suas animadoras de torcida e os técnicos depois de vencerem um jogo do campeonato estadual. Quando o ônibus enguiça num trecho isolado da estrada, a euforia da vitória logo dá lugar à sensação de mau pressentimento. Notícias sobre horrores inomináveis surgem através do rádio. Não há nenhuma possibilidade de ajuda à vista, os celulares estão sem sinal e seus pedidos de socorro pelo rádio ficam sem resposta. Quando o Sol se põe, a Criatura ataca o ônibus, levando a motorista e os técnicos como suas primeiras vítimas. Os adolescentes, aterrorizados, terão de se defender sozinhos do predador de apetite insaciável. A Criatura faz suas escolhas entre os atletas, um a um, farejando a próxima vítima. Os jovens se vêem diante de um terrível dilema: devem ficar todos juntos no ônibus, ou devem fugir e abandonar aqueles que foram marcados para morrer? Finalmente, surge uma resposta através do rádio: é Taggart. Consumido pelo desejo de vingança, ele tem seguido a pista do monstro e está a caminho. Mas Taggart será páreo para o monstro bizarro que levou seu filho mais novo?
