His error was not setting the Altimeter correctly for the airport he was doing the demo at. I can't remember all about the ejection but his canopy didn't fully deploy and he landed in the fire ball. However his seat landed first putting out the flames right were he landed. He was also wearing a nomex flight suit. Contributing to his survival, all this happened in the space of a few seconds to boot.

As a result the jet stalled and couldn't recover. He just wasn't paying attention to all the details and got distracted, which can happen, but there is no room for error in that business. BTW, he did survive; ejection seats are made to get pilots out of trouble very fast, so he was launched back and away from the crash.

That F-16 exploded in a huge fireball and the pilot may have survived the ejection only to land in the fireball. Other pilots eject at the wrong angle - enough said. And 2 pilots died in S.Africa recently when a training flight through a mountain pass went horribly wrong. One pilot ejected into the cliff face and the other ejected right into an oncoming truck and ended up in the driver's cab. If I'm not mistaken several motorists were also injured in the ensuing pile-up.
