    In many places this has happened. REALLY..
    Even in the USA..
    1 place had 1 room that was Not setup correctly, and the Janitor was pulling the plug, and no one noticed for 3 years, why people were ALWAYS dying in that room. Think they had 3-4 deaths, before SOMEONE caught on.

      Actaully happened in South African hospital - 2 or 3 times in the same IC ward, until they found out the common theme of the same bed being too much of a coincidence
