The plane was overweight, in a tailwind, missing an autoalignment spacer, and the flight engineer shut down an engine without the captain's permission--- take out any number of these aspects, and the aircraft might have been able to be flown out of the tireburst situation; or avoided it altogether

    They did change the tire as part of Concorde's return to flight. The Goodyear tires which had a tendency to fragment, were replaced with Michilen tires which were designed not to fragment. Runway checks for every flight are impossible given the number of takeoffs every day. Runways are inspected on a daily basis for FOD (Foreign Objects Damage). However, this particular piece of metal fell off between the regular inspection cycles.
