Boeing 727 trijet was also fine. DC-10 did have problems in the beginning (like the cargo door lock), but it later became one of the safest jets.

The Goodyear tires which had a tendency to fragment, were replaced with Michilen tires which were designed not to fragment. Runway checks for every flight are impossible given the number of takeoffs every day. Runways are inspected on a daily basis for FOD (Foreign Objects Damage). However, this particular piece of metal fell off between the regular inspection cycles.
