dude getting hit in the face in general hurts and in a grapple situation its not easy to hold someone down and punch them at teh same time, but the punches help to wear the person down. pressure points arnt really something you can truly aim for in a real fight unless your fighting an amatuer or an unmissable opportunity presentrs itself.
