    As long as his wrist remains in the same possition, he won't have much trouble with them unless he begins to massage using wrist motion then that would be a problem to the point of RSI/RMI begins.

    I do have some objections onhis leg position. This move is better done kneeling in seiza on one side of the body and leaning the full body to be align as much as possible with that of the patient. This will result in less contact and less ambiguous (that is leg and groin) contact. The pressure is just right it does not hurt usually but it would be good to see the patient's face, it would show her feedback. Nice video and good form!
