    To be put simply it's usually caused by the person working on the equipment hot. The person working on it usually creates a phase-phase or phase-ground situation and, well you get the picture. 6000 degree molten copper, toxic gas...you might be unlucky and survive.

    You really don't want to mess with that stuff. I work for an Irving owned company and if we are ever in a situation where we have to work of anything live, we wear this big shiny arc flash suit and require special training. Always work on de-energized equip, even if it means coming back another day

    Wow. I deal with 480V 150A circuits at work all the time. I build motor and heater control panels with lockout switches, but occasionally have to work on them live. No PPE, No training...and I wear poly cotton shirts. Guess my life will be pretty short. How often does this sort of thing occur? Wouldn't you have to be sticking something in there in the first place?
