Arc Flash Accident - Live Cam

we watch this clip in our safety class at the power utility co. acording to the instructor, the fist guy obviously the one working crosed phased a wrench no P.P.E used and suffored seriously.

I had to re-watch this several times. I didnt see the guy at the bottom at first.
Obviously he was crossing the deadfront zone into live gear and shorted some buss possibly. Lots of rules being broke here.

1. Need much higher level of P.P.E. per NFPA 70E. (Full arc blast gear: head/chest/back gear with tinted face shield, one piece). Flash proof heavy duty bibs. Westex fireproof bibs, shirt, pants beneath that. Heavy duty long sleave high volt gloves, safety glasses.

2. Only one man in the zone. Notice they are leaning on grounded steal.

Any ideas to why this breaker exploded? internal Phase to phase? Phase to ground?

You know that is exactly what happened to my dad last year, his face got burned really bad, but he's ok now.

Yes he did die the next morning, i just finished a flash arc class at work and this video was a big topic.
You can see his pants ignite again after he runs away, what you don't see is that his hard hat is melted to his head. The other guys were burned too, one was in the hospital for a few months after.Remember that an arc flash happens in less then a second no one has time to react thats why they should of used protective clothing and equipment to reduce the chance of a 3rd. deg. burn.

im an electrician and arcs arent fun at all. i have come close to one but cut the power, i was lucky it wasnt an arc flash. these arent joking around things. dont go out ther and try to do something stupid

No. He didn't die. He was in a coma for two months though unfortunately. That is a perfect example of Arc Flash and how dased an individual can be after the blast.
